YOUTH WAIVERS FOR MUFA LEAGUES

You must be at least 14 years old on the first day of a given league to participate, or by the date specifically listed on the page for that league.

Youth waivers (click here) required for participation in all MUFA leagues and events. Be sure to fill out both forms and send them to:
